3Magkuha ug Mayor’s Permit sa MPDC dala ang sketch map (Secure Mayor’s Permit at MPDC bringing with you the sketch map. Person Incharge: MPDC Personnel])30 min. - 1 hour
4Balik sa Nasipit Water District Office dala and mga requirements ug ihatag sa Information Desk Personnel (Go back to the Nasipit Water District Office bringing with you the requirements & submit it to the information desk personnel)10 min.
5Magfill-up sa Application Form (Fill-up the Application Form)2 min.
6Ihatag ang Application Form sa Information Desk Personnel para sa Orientation (Submit the Application Form to the Information Desk Personnel for Orientation)10 min.
7Magbayad sa Installation Fee sa Cashier dala and Application Form ug lista sa mga materyales nga palitonon (Pay the installation fee to the Cashier bringing with you the Application Form & List of materials to be purchased)5 min.

    4Magpaskedyul sa Service Connection Evaluation [para sa dugay nang naputlan] (Ask for Evaluation Schedule)5 min.
    5Maghulat sa balay para sa pagtaod sa Service Connection (Wait for the re-opening of the service connection)4 hrs. - 1 day
